Several case studies are described illustrating noncompliance with the design process, for the purpose of emphasising the critical steps in the process. The use of a risk approach in design is dealt with, showing that risk can be a satisfactory design criterion, and that identification of the consequences, and hence quantification of risk, can lead the way creation of value for a mining operation.
